for Dementia Family Caregivers
Service Description
We “CARE“ Connection: A Dementia Caregivers Support Group 6 Month Support Program 12 Sessions Total 1 online session per month (1 st Wednesday of the month) 1 in-person session per month (3 rd Wednesday of the month) Informal respite care is available for loved ones with dementia during the 6 in-person sessions, if needed Support Sessions Emphasize peer support and community Highlight empowerment in the care partner relationship Monthly In-Person Sessions o Research-based, psycho-educational, skill-building o Focus on both the positive aspects and struggles of caregiving Online Sessions o Resource-based, Q&A format Monthly themes: 1. Personal identity and the role of the carer 2. Stress and the emotional side of caregiving 3. Challenges of Caregiving 4. Caregiver wellbeing: quality of life, resiliency & self-care 5. Caregiver wellbeing: self-care and making the positive aspects more positive 6. Caregiving and dementia resource sharing Caregiver Connection Cafe o The latter part of every session o A social time to connect with other caregivers Memory Lane Social Group Informal respite for loved ones living with dementia In the same building, at the same time as the caregiver support program (*a caregiver must be registered for, and attending the support program toutilize MLSG)
